When started from a command line it should show an error message if it doesn't want to start.

The strange thing is that every time I double click the Manager, although nothing happens, it appears in Windows' Task Manager... I tested it again on a PC with WinXP and it ran just fine. When I tried to execute through the console, I didn't get an error but the console buffer stopped at the apache thing as as far as I remember... I 'll check again and report back.

I thought you knew what you were missing when saying:

I've updated MDM so that when the 'Linked to Note' checkbox is checked it automatically sets Alt/Pressroll note to the same value as Note.

It doesn't say that I've added any new buttons. It is just a change in behaviour - now when you check the 'Linked to Note' checkbox it sets Alt/Pressroll note to the same value as Note to prevent from what happened with to you before.
